As per preliminary inquiries, the forms fell from M Jyothi's vehicle, who was serving as the supervisor for the survey in the Kapra Mandal area. She has been suspended with immediate effect for dereliction of duty. Additionally, a show-cause notice has been issued to the Municipal Commissioner of Jawahar Nagar Municipal Corporation as it failed to ensure proper monitoring and transport of the survey forms in trunk boxes. The Municipal Commissioner has assured the public that necessary measures are being taken to prevent similar lapses in the future.
